Request for Service

Each school district will complete and submit an annual District Request for Service. The purpose of the DRFS is to identify the services being requested in all three tiers for the coming school year. A SET-BC service coordinator will meet with each district planning team in May, June or September to facilitate completion of the District Request for Service form. ​ District teams are not required to complete the DRFS prior to the meeting – the SET-BC consultant will ensure it is completed during the meeting itself.  However, the district team should come to the meeting prepared to list their Tier 1 Pro D requests, Tier 2 training requests, Tier 2 classroom-based solution requests (based on annual allocation), Tier 3 specialist training requests and Tier 3 student-based solution requests (based on annual allocation).

The deadline for District Requests for Service is September 30th for that school year.

The SET-BC service delivery process is supported by a number of tools and forms. Depending on the service delivery component, some of these documents are available to districts as samples and some are used by SET-BC during the service planning and implementation process. This application form was used by the SET-BC provincial team in recent years when coordinating and supporting classroom-based technology projects.  It is included here as a SAMPLE as many districts used it as a starting point when designing their own Tier 2 classroom-based solution screening/selection process.

This is the standard District Screening Checklist that many districts have been using for many years to screen/select students for whom they are requesting SET-BC services.  It is included here as many districts are continuing to use this document for their Tier 3 screening/selection process.

This is the form that will guide the annual District Request for SET-BC Service (DRFS) meeting.  It provides information on the services the district is requesting for the coming school year in all three tiers.  It also contains the District SET-BC Service Agreement and Terms of Service that requires administrative signature to initiate the SET-BC service delivery process.

This document provides a template for the Tier 1 Pro D, Tier 2 Training, Tier 2 Classroom-based Solution, and Tier 3 Student-based Solution Collaborative Action Plan (CAP) meetings.  This generic form will be completed during the CAP meetings between the SET-BC consultants and the key team contacts for each service request approved during Provincial Review.  The annual deadline for completion and submission of CAP forms is October 31st for that school year.

Towards the end of each school year, prior to the Annual District Request for Service meeting, the district team will be sent a link to an online survey requesting feedback and information on the SET-BC services provided during that school year.
